The Akatsuki Sasuke Costume
The Akatsuki (Teenager) Sasuke started to appear in the Naruto Shippuden Series, when he was 16 years old, and is training under Orochimaru. He continues to don the same outfit, adding the black cloak with red clouds when he entered the Akatsuki, an organization for renegade ninjas.

Upper Part of the Teenager Sasuke Costume
The teenage Sasuke wore a white Karate Uniform, the upper part, with the chest part open. He sometimes take it off and let it just hang over his shimenawa belt (more on that later).
When he entered the Akatsuki organization, we wore the Akatsuki cloak over it. Someimes, he wore just the Akatsuki uniform and nothing under it.

Lower Part of the Teenager Sasuke Costume
The Naruto Shippuden Sasuke wore black Kung Fu pants and black ninja tabi shoes.
Now if you want the Sasuke that does not have the black cloak on, you have to wrap a black cloth around the pants then tie it with a purple rope.

Accessories of the Teenager Sasuke Costume
Now for the Accessories.
You are going to need the Kusanagi sword, a black wrist protector glove and a ninja forehead protector (anti-Konoha).
The Kusanagi sword and the headband is quite hard to find in physical stores (I checked) so, you might want to grab it from here.
The Kid Sasuke
On the early series of the Naruto manga and TV animation, the young Sasuke wore a costume that is rather really easy to emulate.
The Shirt and the Shorts
OK, just grab a short sleeved sea blue shirt or a long sleeved black shirt (these are rather very easy to find in physical shops, if you are wary with online shopping) and sew the Uchiha crest found at Amazon stores at the back. Pair it up with a plain white shorts (another easy-to-find item).
Accessories of the Sasuke Costume
The white sleeves (on both arms and legs) -- you can easily make it using a white clothe. Just sew the ends together!
Grab this blue ninja shoes, Konoha forehead protector and a kunai set and you are ready to go. You can also add a cursed seal on your neck!
